DGO Telework Agreement

Instructions for filling out the form:

BEFORE YOU START: Be sure to fill out all the required fields on the form below. Any field marked with a red asterisk (*) is a required field. Any required fields left blank will prevent the form filler from submitting the form. Once all required fields have been filled out, a blue submit button will appear on the bottom of the field window allowing you to submit the form for processes.

Step 1: Fill in all of the required fields within the DGO Telework form below.

Step 2: Once you have filled out ALL required fields, a blue “Click to Sign” button will appear at the bottom of the form window.

Step 3: A pop-up window will appear asking for your supervisor’s information. Enter your supervisor’s first name, last name, and email address, and then click on Next.

Step 4: Another pop up window will appear asking for YOUR email address. This email address will be used to send a confirmation email that’s required to complete your submission.

Step 5: Check the inbox for the email address you provided in Step 4. You should receive an email from Adobe Sign asking you to confirm your email address. Click on the “Confirm my email address” link within the email to complete the process of submitting your form.

Step 6: You should then be redirected to the completion page, informing you that you have successfully submitted the form.
